1. Acceptance of Terms
By scheduling or participating in virtual consultations with Juliana Iizuka, FNP through Advance Telehealth, you agree to these Terms & Conditions. These terms may be updated at any time.
2. Services Provided
Virtual consultations include medical evaluations, treatment recommendations, prescription management (where legally permitted), and health education via secure video/audio platforms. Services do not constitute emergency care.
3. Payment Terms
Services priced $55-$75 per visit, payable at time of booking
Payment via credit card or HSA/FSA accepted
Superbills provided for insurance reimbursement
No refunds after consultation completion
Cancellations within 24 hours may receive partial credit
4. HIPAA & Privacy
All platforms are HIPAA-compliant with end-to-end encryption
Patient information never shared without written consent
Secure electronic prescribing to local pharmacies
Records maintained per state and federal regulations
5. Prescription Policy
Most medications can be prescribed electronically
DEA-controlled substances require state-specific protocols
Prescriptions sent directly to your chosen pharmacy
Refills available through follow-up consultations
6. Limitations of Virtual Care
Not a substitute for emergency medical services
Physical exams limited to visual/telephone assessment
In-person evaluation recommended for complex cases
No diagnostic testing performed during virtual visits
7. Cancellation & No-Show Policy
24-hour cancellation notice required
No-shows forfeit prepaid appointment fee
Chronic no-shows may require prepayment
8. Patient Responsibilities
Provide accurate complete medical history
Maintain private, quiet consultation environment
Ensure stable internet connection
Follow prescribed treatment recommendations
9. Governing Law
These terms governed by laws of the state where services provided. Disputes resolved through binding arbitration.
